Supported Decision-Making (SDM) is a person-centered planning and person-driven decision-making model that can be used as an alternative to guardianship, to help individuals retain their legal rights. SDM emphasizes personal choice based on information and advice from trusted advisors.    

This legal basics webinar will explore how SDM can be used to preserve the fundamental human and constitutional rights of persons with disabilities, including older adults who are experiencing changes in memory and cognition. We will explore the fundamentals of the PRACTICAL tool
that guides users through SDM as an alternative to avoid or replace guardianship. Lastly, we will talk about the state of the law on SDM, and integrating SDM principles into advance care planning.
